Individuals with intellectual and developmental disabilities (I/DD) often experience fragmented healthcare systems that separate medical care from the supports that have the greatest impact on their daily lives. This presentation explores a different approach: a model of care that places Home and Community-Based Services (HCBS) providers at the center of care coordination, leveraging their trusted relationships, day-to-day engagement, and deep understanding of individual needs. We will discuss why HCBS providers are uniquely positioned to improve health outcomes, enhance the member and family experience, and bridge the longstanding divide between healthcare and long-term supports. Attendees will gain insight into a provider-led, person-centered model designed to deliver better outcomes, greater accountability, and a more integrated experience for individuals with I/DD and their families.
